WHAT IS PELLET THERAPY & WHY SHOULD YOU CONSIDER IT?

What is Pellet Therapy and why should you consider it? Subcutaneous estrogen replacement therapy often referred to as “Pellet Therapy” is not a novel idea. It was initially described in 1938 as an alternative treatment for menopausal symptoms. Subcutaneous estrogen replacement involves the placement of crystalline, biodegradable pellets of estradiol subcutaneously into the buttocks. Placement is performed in the office using local anesthesia.

Advantages of Pellet Therapy

Estrogen replacement therapy (ERT) is very advantageous. ERT is superior to any other remedy for the treatment of hot flashes and vaginal atrophy. It has also been found to be cardio-protective, to improve cognitive effects, and to aid in the prevention of osteoporosis.

Despite these advantages, many patients discontinue their ERT for various reasons. The most common include irregular bleeding and or concern about breast cancer. Women discontinue therapy due to unreliability in taking pills, skin reactions to patches, nausea, or incomplete relief of symptoms. These patients may benefit from alternative routes of ERT like “pellet therapy”.

Subcutaneous ERT, unlike oral estrogen therapy, is better able to mimic premenopausal physiologic levels of estradiol and estrone. Subcutaneous estrogen has a favorable effect on lipid profiles with an increase in “good” cholesterol HDL and decreased level of LDL’s “bad” cholesterol. Although estradiol levels differ among patients, individual levels within each patient have very little variation. Implants provide more stable circulating estrogen levels than any other route of administration.

Insertion

Insertion is easily performed in the office. After a small area of skin is prepared with local anesthesia, a 5 mm skin incision is made in the subcutaneous fat of the buttocks. The trocar with cannula is advanced through the incision. The trocar is removed and the pellets are loaded into the barrel of the cannula. The obturator is inserted forcing the pellets out of the instrument into the subcutaneous layers of the buttocks. Complications, albeit rare, include bleeding from the insertion site, bruising, or wound infection.

Testosterone & Progesterone Therapy

Another potential benefit of pellet therapy is the ability to combine subcutaneous estradiol and testosterone replacement. Testosterone therapy has been used for patients who complain of reduced libido. A combined dose of estrogen and testosterone has been shown to significantly improve libido, sexual enjoyment, and menopausal symptoms lasting for 5-6 months.

Appropriate patients include peri/postmenopausal women seeking to relieve hot flashes, vaginal atrophy, maintain or increase bone density, and produce favorable lipid changes. Patients with an intact uterus require progesterone therapy to protect the endometrium, as do patients using other forms of estrogen replacement.

Pellet Therapy and Libido

Subcutaneous ERT or “Pellet therapy” is a convenient form of estrogen replacement therapy, eliminating the need for daily pill or patch. Furthermore, combined therapy adding testosterone can also be used to adjudicate symptoms of decreased libido, leading to more sexual enjoyment.

Part 2: How Do Bio-Identical Pellets Work?

Bio-identical pellets are made of an inert substance that holds it together along with either bio-identical Testosterone or Estradiol. If you require Estradiol it is one small pellet. Men, and also women, require Testosterone. There are different dosages (mg) of Testosterone pellets (25 mg up to 200 mg) and which mg pellet and how many Testosterone pellets you receive depends on the end dose that you require. What you need to know before taking a vitamin or mineral supplement.

The average American diet leaves a lot to be desired. Research finds our plates lacking in a number of essential nutrients, including calcium, potassium, magnesium, and vitamins A, C, and D. It’s no wonder that more than half of us open a supplement bottle to get the nutrition we need. Many of us take supplements not just to make up for what we’re missing, but also because we hope to give ourselves an extra health boost—a preventive buffer to ward off disease.

Getting our nutrients straight from a pill sounds easy, but supplements don’t necessarily deliver on the promise of better health. Some can even be dangerous, especially when taken in larger-than-recommended amounts.

How Much of Each Nutrient Do You Need?

Here are the recommended levels of daily intake for several important nutrients.

The Excitement Over Supplements

We’ve heard a lot of encouraging news about supplements. A series of studies hailed vitamin D as a possible defense against a long list of diseases, including cancer, diabetes, depression, and even the common cold. Omega-3 fatty acids have been touted for warding off strokes and other cardiovascular events. And antioxidants such as vitamins C and E and beta carotene were seen as promising silver bullets against heart disease, cancer, and even Alzheimer’s disease.

Here’s the big caveat: Many of those exciting supplement studies were observational—they didn’t test a particular supplement against a placebo (inactive pill) in a controlled setting. The results of more stringent randomized controlled trials haven’t yielded the same good news.

“Often the enthusiasm for these vitamins and supplements outpaces the evidence. And when the rigorous evidence is available from randomized controlled trials, often the results are at odds with the findings of the observational studies.”

– Dr. JoAnn Manson, Chief of Preventive Medicine at Brigham and Women’s Hospital, Professor of Medicine at Harvard Medical School, and Principal Investigator of a large randomized trial known as VITAL (Vitamin D and Omega-3 Trial).

Because observational studies may not fully control for dietary factors, exercise habits, and other variables, they can’t prove whether the treatment is responsible for the health benefits. “People who take supplements tend to be more health conscious, exercise more, eat healthier diets, and have a whole host of lifestyle factors that can be difficult to control for fully in the statistical models,” Dr. Manson continues.

Diving Deeper

Some supplements that were found to have health benefits in observational studies turned out, with more rigorous testing, to be not only ineffective but also risky. Vitamin E, which was initially thought to protect the heart, was later discovered to increase the risk for bleeding strokes. Folic acid and other B vitamins were once believed to prevent heart disease and strokes—until later studies not only didn’t confirm that benefit but actually raised concerns that high doses of these nutrients might increase cancer risk.

How to Get Your Nutrients

We need a variety of nutrients each day to stay healthy, including calcium and vitamin D to protect our bones, folic acid to produce and maintain new cells, and vitamin A to preserve a healthy immune system and vision.

Yet the source of these nutrients is important. “Usually it is best to try to get these vitamins and minerals and nutrients from food as opposed to supplements,” Dr. Manson says.

What Are The Best Sources?

Fruits, vegetables, fish, and other healthy foods contain nutrients and other substances not found in a pill, which work together to keep us healthy. We can’t get the same synergistic effect from a supplement. Taking certain vitamins or minerals in higher-than-recommended doses may even interfere with nutrient absorption or cause side effects.

How Dr. Manson Weighs Supplements

Before you take any supplements for disease prevention, it’s important to know whether the potential benefits outweigh the risks. To make that conclusion, you need to look at the results of well-designed studies. A recent randomized trial in men suggested multivitamins have possible benefits for cancer prevention. For many of the other popular supplements, including vitamin D and omega-3 fatty acids, results from randomized controlled trials should be available within the next five years, according to Dr. Manson.

Until then, be judicious about your use of supplements. If you’re lacking in a particular nutrient, ask your doctor whether you need to look beyond your diet to make up for what you’re missing—but don’t take more than the recommended daily intake for that nutrient unless your health care provider advises it.

“Mark”, a 48 year old gentleman came to my office complaining of fatigue, an increase in aches/pains, an increase in irritability, and a decreased ability to build muscle. He still has a good libido but had noticed a decrease in morning erections. “Mark” had seen his regular doctor who told him his labs were normal and that he needed to exercise more but he was too tired to do that. He knew there was something else going on. When NLW doctor reviewed his symptoms and lab results, we both agreed that he suffered from low Testosterone. At first, “Mark” was concerned about the risks associated with replacing Testosterone based on recent news releases. We went over the study that the news was citing. The news reports were telling men that Testosterone replacement increased heart disease risk.

However, the study being cited had tested older, high heart disease risk men whose Testosterone level was never in an optimal range and they were treated for too short a time to even show a benefit. Optimal Testosterone levels are around 600-1400 depending on the male, men in this study were averaging levels in the 300 range. This was a poorly done study, and it is a shame that it was used to scare men away from seeking help.

In fact, there are many studies showing that when Testosterone levels are optimized and given enough time to work, that Testosterone actually is protective for the heart as well as the brain. Remember that the normal Testosterone lab values are not optimal–they are simply the expected values based on testing the male population—both ill and healthy. You want optimal levels if you want a life of vitality.

Optimizing a man’s Testosterone level involves replacing his Testosterone along with starting healthy lifestyle changes. Which form of Testosterone replacement a man chooses depends on the individual.
